History provides access to all your past transcription sessions and their summary notes. Use it to continue editing summaries, reference previous encounters, or clean up completed sessions.

What appears in history

Each session is listed as a single item with key metadata:

Avoid entering patient identifiers (names, MRN, DOB) into custom labels. Use your electronic medical record system for patient identification and linking.

Viewing your sessions

Sessions are listed in reverse chronological order, with the most recent at the top. Navigation:
  • Scroll to browse older sessions
  • Use search or filters (if available) to find specific encounters
  • Select a session to view its summary note

Editing past notes

You can return to any summary to continue editing:

1. Open the session

  1. Navigate to History
  2. Select the session you want to edit
  3. The summary note opens for review

2. Review and edit

Follow the same review workflow as for new summaries:
  • Read the entire note
  • Fix transcription errors
  • Verify clinical safety items
  • Edit for completeness and clarity
If you’re returning to a note hours or days later, pay extra attention to time-sensitive details like lab results, medication changes, and follow-up plans. Your memory may be less fresh.

3. Save changes

Save your edits. The session status updates to reflect your changes. Review the full editing workflow →

Deleting sessions

Remove sessions you no longer need from your working list.

Before deleting

Confirm that you have:
  • ✓ Completed all required documentation in your medical record
  • ✓ Saved any information you need to retain
  • ✓ Met your organization’s record retention requirements

How to delete

  1. Select the session you want to remove
  2. Choose the delete option
  3. Confirm the deletion
Deleted sessions cannot be recovered. Ensure you’ve saved all necessary information to your medical record before deleting.

Best practices

Regular maintenance

  • Review and finalize summaries within 24-48 hours while the encounter is fresh
  • Archive or delete completed sessions weekly to keep your list manageable
  • Use consistent labeling conventions for easier searching

Audit trail

Your electronic medical record system should serve as the official audit trail for clinical documentation. Quiver Transcribe history is a working tool, not a permanent record.
